The Interim Care program provides short-term medical evaluation and treatment services. Our goal is to ensure patients do not go without care for manageable conditions during their transition to a permanent primary care provider.
Who is eligible?
- Residents of Queen Anne's County
- Adults 18 years of age and older
- Individuals who do not currently have a primary care provider (PCP)
- Individuals with a known or suspected diagnosis of high blood pressure, type 2 diabetes, and/or high cholesterol
How we help:
- Provide short-term medical care for the conditions listed above
- Assist with finding and transferring to a permanent primary care provider
- Connect patients with helpful community resources
- Note: Interpretation and translation services are available.

Primary Care Extension Program
This nurse practitioner-led program offers short-term, intensive chronic disease support for individuals with an established primary care provider who require additional time, support, and education to manage their health conditions.
*3-6 months per client (variable and individualized)
