Interim Care

​The Interim Care program provides short-term medical evaluation and treatment services. Our goal is to ensure patients do not go without care for manageable conditions during their transition to a permanent primary care provider.

Who is eligible?

  • Residents of Queen Anne's County​
  • Adults 18 years of age and older
  • Individuals who do not currently have a primary care provider (PCP)
  • Individuals with a known or suspected diagnosis of high blood pressure, type 2 diabetes, and/or high cholesterol

How we help:

  • Provide short-term medical care for the conditions listed above
  • Assist with finding and transferring to a permanent primary care provider
  • Connect patients with helpful community resources
  • Note: Interpretation and translation services are available.

Primary Care Ex​tension Program

This nurse practitioner-led program offers short-term, intensive chronic disease support for individuals with an established primary care provider who require additional time, support, and education to manage their health conditions.

*3-6 months per client (variable and individualized)
