Our team works with you in the comfort of your home to assess your needs, help you reach your personal health goals, and connect you with the right community resources—at absolutely no cost to you.
How It Works
Referrals to the MICH program are typically identified through 911 calls or other community sources. Participation is completely voluntary. Once a referral is made, a member of our team will contact you to arrange a convenient time for a home visit.

- Adults ages 18 and older
- Residents of Queen Anne's County
What to Expect During a MICH Visit
Based on your individual needs, a home visit may include:
