Assessment, Evaluation and Review Services

​​AERS is a Maryland program based in local health departments that provides assistance to frail, aged, and disabled adults and children. Our goal is to create personalized care plans and connect clients to community resources, promoting independence and preventing unnecessary entry into a nursing home or other residential care facility.​


Who AERS Can Help: We evaluate Maryland residents who need support to remain living safely in the community. This includes individuals who:

  • Are at risk of being placed in a care facility or face a change in their normal living situation.
  • Have chronic medical conditions.
  • Need help with activities of daily living, such as bathing, dressing, meal preparation, or mobility.


What to Expect:

  • Comprehensive Evaluation: A licensed Registered Nurse (RN) will conduct an on-site evaluation (in the client's home) to assess the individual's needs.
  • Personalized Care: The RN will develop a tailored plan of care to keep the individual in the least restrictive environment possible.
  • Resource Connection: We will identify and connect you with available community services.
  • Note: Interpretation and translation services are available.


Programs We Can Help You Access: Depending on your evaluation, you may qualify for specific Maryland support programs. (Note: Individuals must meet specific medical and financial criteria, such as Medicaid eligibility, for these services).

  • Community First Choice (CFC): Provides assistance with activities of daily living, such as bathing, dressing, meal preparation, light chores, and accessing the community.
  • Community Personal Assistance Services (CPAS): Provides assistance with activities of daily living, such as bathing, dressing, grooming, meal preparation, and light chores.
