Adult Evaluation and Review Services (AERS)

Adult Evaluation and Review Services (AERS) is a Maryland Medicaid program that provides evaluations for older adults and disabled adults and children. Evaluations are done by a Registered Nurse and/or Social Worker in the person’s home or other location in the community.

Our Carroll County AERS team will assess the person’s medical, functional and psychosocial needs. We then create a person-centered plan of care to help them live optimally in the least restrictive setting possible. We can help connect them to additional services and assess their eligibility for Medicaid programs.

AERS also performs evaluations for the federally mandated program, PASRR (Pre-Admission Screening and Resident Review), for clients who have an Intellectual Disability (ID) or Mental Illness (MI).​​​​
