Assessments, Evaluation and Review Services (AERS)

Assessments, Evaluation and Review Services (AERS) is a Maryland Medicaid program that provides in-home and community-based evaluations for older adults, individuals with disabilities, and children.

Evaluations are conducted by a Registered Nurse and/or Social Worker and focus on each person’s medical, functional, and psychosocial needs. Based on this assessment, the Carroll County AERS team develops a person-centered plan of care designed to support individuals in living as independently as possible in the least restrictive setting. The team also helps connect individuals to appropriate services and determines eligibility for Medicaid programs.

AERS also completes evaluations for the federally mandated PASRR (Pre-Admission Screening and Resident Review) program​ for individuals with intellectual disabilities or mental illness.

In addition, AERS conducts evaluations for several programs within the Carroll County Bureau of Aging & Disabilities​​, including: SCP, COW, CFC, CPAS, and SALS.
