Birth Certificates


The Allegany County Health Department can issue certified copies of birth certificates for individuals born in Maryland from 1940 to the present.  Birth certificates may only be obtained by individuals named on the certificate or by a representative of the individual with proper court documentation.

Same Day Service:
As a courtesy to those born in Maryland, the health department can issue same-day certificates on a walk-in basis.  You must present a valid government-issued photo ID (driver's license, U.S. passport, MVA photo ID card, or military photo ID). The photo ID must have an issue and expiration date. If your ID has expired, please call for additional information.  The fee is $20 per certificate.  We accept cash, local checks, Visa, MasterCard, and Discover.

Order Online:
To order online, visit www.vitalchek.com.  Certificates are processed in Baltimore and mailed to the applicant's address.

Order by Mail
To order by mail, applications can be obtained at the health department or downloaded at https://health.maryland.gov/vsa/Pages/birth.aspx.  You must complete the application and mail it to the Division of Vital Records in Baltimore at the address listed on the form ​to be processed.

Armed Forces Member
With proof of military service, there is no fee for a birth certificate for a current or former armed forces member when the certificate is requested by that member. 

Births Prior to 1940
Birth certificates for birth years prior to 1940 must be requested from the Division of Vital Records.  For details, visit https://health.maryland.gov/vsa/Pages/birth.aspx or call 410-764-3038.
