For Schools and Child Care Facilities
ImmuNet is Maryland’s Immunization Information System (IIS). It is a secure web-based registry operated by the Center for Immunization at the Maryland Department of Health (MDH). ImmuNet information is confidential and available only to authorized users.
Public school nurses can print pre-filled MDH 896 School Certificates from ImmuNet.
Note that parents/guardians can access their child(ren)'s school certificates directly via the MyIR portal here. MDH provides these MyIR brochures for printing and distribution, or complete the Brochure Request form (here) to have brochures mailed to you.
Per State Statute §6–303(b), results of blood tests for lead poisoning are also available in ImmuNet.
