If you have a notifiable condition case to report, please follow the instructions for the specific condition. 

Occupational Health

If you are a provider reporting a known or suspected case of occupational disease or injury, submit this form or call the Environmental Health Helpline at 1-866-703-3266.

Workers can file complaints with the Department of Labor (call 1-888-257-MOSH for emergency or life-threatening issues in the workplace).

Foodborne Illnesses, Manufactured Foods, Milk and Dairy, and Cottage Foods

To place a complaint about foodborne illness, Maryland commercially manufactured food (pre-packaged foods purchased at a grocery store), Maryland milk or dairy products (bottled milk, yogurt, cheese, ice cream etc.), Maryland food or milk and dairy processing/manufacturing facility, or a cottage food product (a home-prepared or packaged food product), please complete this Complaint Form.

Syphilis and HIV Referral Hotline

To place a complaint about a retail food store or other retail food service facility (Restaurant, Carryout, Grocery Store, etc.), please contact the local health department in the county where the facility is located.
