H.R. 1, enacted into law on July 4, 2025, created a $50 billion fund called the Rural Health Transformation Program (RHTP).
On December 29, 2025, Maryland was pleased to receive a notice of award from the Centers for Medicare and Medicaid Services (CMS) for $168,180,837.61 for budget period one, of five, of the Rural Health Transformation Program. Maryland is currently in discussion with CMS regarding approved programs and adjusted budget details.
Updates will be posted on this page as available, including opportunities to apply for the RHTP Committee and to submit proposals for competitive subrecipient funding. Please continue to follow for more information.
CMS posted a Notice of Funding Opportunity for states to apply for the RHTP on September 15, 2025.
To gather feedback to inform the proposal, the Department:
Released a Request for Information (RFI) invitation on September 9, 2025. The RFI closed on September 30 and the Department received over 325 responses.
Held an introductory public webinar on September 15, 2025. Slides are available here.
Collaborated with local health departments to organize 17 local listening sessions held in-person and virtually throughout rural Maryland. More than 250 people participated.
On October 20, 2025, the Department presented a summary of the feedback and an outline of our draft Rural Health Transformation Plan. Slides are available here.
Maryland submitted an application by the deadline posted.
