The Board will be closed for business on the state holidays listed on the Department of Budget & Management website. The Board will re-open the following business day.

When Coming to the Office to Renew Your License / Certification

We do not accept cash. Acceptable forms of payment are as follows:
 
RN / LPN / Advanced Practice Renewal
 
Personal checks, money orders, Visa and MasterCard credit or debit cards will be accepted. You will be required to have a valid photo ID or drivers license when renewing in person.
 
Nursing Assistant Renewal
 
The Nursing Assistant Certification division accepts Visa or MasterCard credit/or debit cards, money orders or certified checks ONLY. You will be required to have a valid photo ID or drivers license when renewing in person.

Allow four (4) weeks for processing
  • Incomplete applications and/or nurse practitioner programs that do not comply with 10.27.07.03(A)(4) will require additional processing time.
  • Once issued, the new certification may be viewed and printed from the Board's Look Up a License website.

The Board is asking for your cooperation in facilitating processing of renewal and other licensure applications. We are using forms designed to be scanned into an imaging system to process paper applications. These applications can not be reproduced correctly by your average printer or photocopier. The following licensure applications are NOT available for download. If you need one of these applications sent to you, click on the application type below.

​Clicking on the above application type will  send us an e-mail requesting that application. Include your full name and mailing address in the e-mail. Exam applicants should include the name of the school attended for RN/LPN training. Endorsement applicants should list all states where you are currently licensed and the original state of licensure. 

Please follow the instructions included with your application carefully to assist us in processing your application in a timely manner.​​​​​
