Step 1-
Nurse Practitioner Programs Approved by the Board
The Maryland Board of Nursing must review and approve all Nurse Practitioner Programs before certifying applicants as Advanced Practice Nurses (COMAR 10.27.7.03B(3)(a))
If your Nurse Practitioner program is not listed as "Board Approved" (see below for list), print and submit the "Nurse Practitioner Program Approval Form" to your school. DO NOT complete this form yourself.

Step 2-
Transcripts
· Please have your
school/program mail an official sealed final transcript from your Nurse Practitioner program or have them
sent directly/electronically from your school/program to the following:

***If you attended more than (1) school/program to become a Nurse Practitioner you must submit an official transcript from each program***
Step 3-
Complete a Criminal History Records Check
***Attention Applicants***
Pursuant to sections 8-302(a) and 8-303 of the Health Occupations Article, Maryland Annotated Code, to qualify for a license in Maryland, an applicant must be fingerprinted for a criminal history records check (CHRC). A CHRC includes both a State and national criminal history records check conducted by the Criminal Justice Information System Central Repository of the Maryland Department of Public Safety and Correctional Services, or “Central Repository". The applicant's fingerprints will be maintained by the Central Repository; and all new and additional criminal information will be reported to the Board.
CHRC for APRNs
In accordance with 8-302.1(a)(2) an application for each initial certification specialty is treated as a new application and therefore the applicant is required to be fingerprinted for a CHRC with each new certification specialty if one has not been completed for the Maryland Board of Nursing within the last 12 months.

- You have the option of using a Maryland location or any location in your area to have your fingerprints taken.
- You must request a fingerprint card by calling the CJIS Central Repository in Baltimore City at 410-764-4501 or toll-free number 1-888- 795-0011 to request a blank fingerprint card.
- If you obtain a blank fingerprint card from CJIS, you must have the Maryland Board of Nursing's Agency Authorization and ORI Numbers available as follows.

- Reason: RN/LPN Initial
- Position Applied For: MD Ann. Code Health Occ. §§8-303, 8-304
- Complete the fingerprinting card and take it with you to the fingerprinting site.
- After having your fingerprints taken, be sure to get a receipt. Make a copy of the fingerprint card and attach the copy to your application. Do Not send the original card.
- Mail the fingerprint card and the associated fee to CJIS-Central Repository, 6776 Reisterstown Road, Suite 205, Pikesville, Maryland 21215.

Step 4-
Advanced Practice Registered Nurses include the following categories:
Initial Nurse Practitioner
Certified Nurse Midwife (CNM)
Certified Registered Nurse Anesthetist (CRNA)
Clinical Nurse Specialist (CNS)
(Only applicants scheduled to take their National Board exam are eligible for Graduate Status.)
***Clinical Nurse Specialists (CNSs) wishing to renew their certification for CNS are required to send a copy of their current national certification along with a $10.00 check for the two-year renewal, to the Maryland Board of Nursing, Attention Renewal Department. Until the Maryland Board of Nursing can fund fully online applications, CNS renewal applications must be submitted as a hard copy. We encourage you to continue to use the on-line renewal application feature for your RN. We apologize for any inconvenience. ***
Nurse Psychotherapist (APRN)
***If you are applying for multiple license types, please submit a separate and complete application for each license you wish to apply for. ***
***If you are an Advanced Practice Nurse and will be declaring Maryland or any other NON-Compact State
as your Primary state of residence, you must complete one of the above Advanced Practice applications AND complete the following RN Endorsement application. ***
