Criminal History Records Check

Per § 9-302.1 of the Annotated Code of Maryland, license applicants must undergo a criminal history records check in order to be granted a Nursing Home Administrator (NHA) or Assisted Living Manager License by the Board; this also includes Provisional ​Licenses. The Board may not issue a license if the criminal history record information required under § 9–302.1 has not been received.   

You will need these two numbers to proceed with processing your background check:
