LICENSURE BY ENDORSEMENT

Please obtain the following information:

ü ACTIVE RN/LPN LICENSE from another U.S. STATE

ü a digital passport style photograph in the PNG or JPG file format

ü the 12 digit tracking number below the barcode (on the upper left side) of fingerprint card OR the tracking number on the Livescan receipt (provided in step 2)

ü a Visa or Mastercard credit or debit card

($100 for the permanent license and $40 if you need a temp)

Step One

Request Fingerprint packet and include your name and mailing address from

Step Two

Upon receipt of fingerprint card, have fingerprints rolled onto the card. Please save your 12 digit transaction ID # under barcode for your records

Step Three

Mail fingerprint card and pay associated fees by check or

credit card (Master Card, Visa, and Discover)
