CHW Certification Information

Maryland Community Health Worker (CHW) Certification Key Points:
  • ​A CHW certification is effective for two years from the date issued.
  • There is no fee for certification or certification renewal.
  • Submission of a CHW certification application is completed through the Maryland OneStop portal.
  • The time for an applicant to complete an application is approximately 30 - 60 minutes.
  • The time to determine if an application is complete is within 30 calendar days.
  • The time to determine an application is one (1) to 90 calendar days.

Requirements for CHW certification:
  • Age: The applicant must be 18 years or older.
  • Education: The applicant must possess a high school diploma or GED equivalent.​
  • ​Successful completion of a CHW certification training program accredited by the Maryland Department of Health:
    • The applicant must provide documentation of successful completion of a CHW training program accredited by the Maryland Department of Health.
    • Documentation should be an official certificate, letter, or other document issued by the accredited training program that includes the following five elements:
      • the training program name
      • the training program address
      • the training program accreditation number
      • the date of completion
      • an authorizing signature.

Apply for CHW Certification

Steps to apply for CHW Certification:
  • Submit an application through the Maryland OneStop​ portal for Maryland CHW Certification 
    • Include a form of age verification
    • Include documentation of the successful completion of a CHW training program accredited by the Maryland Department of Health
  • Contact the CHW Team at 410-767-5971 or [email protected] to request a paper application in English or a language other than English.​
