CHW Specialty Training Modules

Community Health Worker Specialty Training Modules​​​

The Community Health Worker (CHW) Program, housed within the Office of Population Health Improvement (OPHI), is pleased to offer CHW specialty training modules to CHW training entities at no cost.​ The available modules cover five specialty health focus areas and are designed to enhance CHW knowledge and skills in these key areas. All modules are available in both English and Spanish.

Each specialty training module includes a PowerPoint presentation, Student Guide, Facilitator Guide, handouts, activities, and a knowledge assessment. The modules are designed to be editable and customizable, allowing CHW training entities to use the materials in whole or in part. The specialty training modules are intended to increase Community Health Worker (CHW) knowledge in key specialty areas so they can better serve their communities.


NOTE: CHWs who complete training through a program that incorporates the materials from these modules may count that training toward the 20 hours of professional development required for CHW certificate renewal.

However, these materials may not be used independently to fulfill the 20 hour professional development requirement for renewal. The modules must be incorporated into an eligible training program.

Requesting CHW Specialty Training Modules

To request one or more of the modules, please complete the CHW Specialty Training Modules Request Form. The modules will be sent to the requestor as a zip file from which the requestor will be able to download and access all related files. 

To improve the utility of future module updates or additional modules, OPHI may request optional feedback (e.g., utility of the module, if/how the module was used) from entities who receive one or more specialty training modules via a survey.
