The Board of Physical Therapy Examiners complies with the Maryland Public Information Act. The public has a right to have access to basic directory information about a licensee that can be viewed here. Personal information may not be disclosed. 

Rosters available for purchase include the following details of active physical therapists and physical therapist assistants: licensee's name, business address, business telephone number, business email, and license number. These rosters are provided in Excel format and are delivered via email to the purchaser. It's important to note that purchased rosters can not be shared or resold by the purchaser.​

• Email the completed Roster Request Form and a copy of the credit/debit card receipt to mdh.bphte@maryland.gov. 
• ​Place "Roster Purchase Request" in the email subject line
• The request can not be processed without a copy of the payment receipt​
