MDS 3.0 Manual Individual Correction/Deletion Requests

The MDS 3.0 Manual Individual Correction/Deletion Request Forms will now be filled out and submitted electronically in iQIES​. 

Providers will need to complete and submit the form for approval in iQIES. State Agency Assessment Coordinators or CMS staff will review and approve the deletion request in iQIES. Once the record is removed from the iQIES database, the status of request will be marked as complete. 

*PLEASE NOTE* this action permanently removes the assessment from the iQIES database and cannot be reversed.

Facility Reported Incidents

*PLEASE NOTE*: The Facility Reported Incident Follow-up Investigation Report has been updated. Use the link below for the updated version of the report.

The Nursing Home Self-Report Form was created for use by facilities to self-report alleged violations of neglect and abuse, including injuries of unknown source and misappropriation of resident property and funds. 

To report a Facility Reported Incident, please complete the Facility Reported Incident Initial Report Form and submit it using the Nursing Home Initial Self-Report Form (Please do not use this form to submit your Follow-up Report).

Once reviewed, a link will be sent to the email provided for submission of the 5-day Follow-up Investigation Report. Please complete the Facility Reported Incident Follow-up Investigation Report and submit it through the link provided in the email.

Nursing Home Emergency Plans:  This template assists nursing homes in complying with COMAR 10.07.02.24.  This sample template is not meant to exclude other formats as long as the facility's plan includes all of the required elements.

​Resident Abuse and Complaint Posters

The OHCQ Resident Abuse and Complaint Posters are available for download below for nursing homes (Health-Gen. § 19-347).  These posters have been approved for use in your facility.  It is recommended that posters are printed in color on 8.5' x 11' size paper.  Posters should be displayed in areas visible to all facility residents, staff, and visitors.
