CTO Information

​

In MDPCP-AHEAD, there is a unique entity type: Care Transformation Organizations (CTOs). A CTO is defined as an entity that hires and manages an interdisciplinary care management team capable of furnishing an array of care coordination ​services to Maryland Medicare beneficiaries attributed to MDPCP-AHEAD participating practices and FQHCs. The interdisciplinary care management team may furnish care coordination services such as: pharmacist services, health and nutrition counseling services, behavioral health specialist services, referrals and linkages to social services, and support from health educators and Community Health Workers (CHWs). 


MDPCP-AHEAD participants have the option of partnering with a CTO every program year. Participants that choose to partner with a CTO will split their program payments in exchange for support from the CTO in providing care management to attributed beneficiaries and assisting participants with achieving care transformation requirements.

CTO Comparison Tool

​CTOs are a key part of MDPCP-AHEAD and provide care management resources, infrastructure, and technical assistance to practices that choose to partner with a CTO. This website page allows practices participating in the MDPCP-AHEAD to view CTOs currently available in their geographic area. Please note that this is PY 2027 information.

  • Program participants will have the opportunity to select a new CTO for the 2027 Program Year during the open enrollment period this summer. Please note that this enrollment period will be hosted in the CMS Portal. More information will be available to participants soon.
