Overview
MDPCP-AHEAD is a Medicare value-based payment program. It’s a voluntary program open to all qualifying Maryland primary care practices and Federally Qualified Health Centers (FQHCs). MDPCP-AHEAD provides funding and support for the delivery of advanced primary care throughout the state. The program supports the overall health care transformation process and allows primary care providers to play an expanded role in prevention, management of chronic disease, and preventing unnecessary hospital utilization. As of January 2026, 460 practices are participating in the program. Here is the PY 2026 Participant List.
