Grant Project Description: 
The program will expand existing service capacity and accelerate access to mental health services on the Eastern Shore by helping to reduce a current two-month waiting list. Grant funds will be used to support a licensed clinical supervisor and a master’s level social work graduate during completion of their required 3,000 hours of supervised clinical social work required for LCSW licensure. The program seeks to serve vulnerable, at-risk low-income residents in Worcester County.

Grant Project Description: 
This project would address obesity prevalence among youth and adults in the jurisdiction through a number of intervention strategies that include online education learning modules; coaching and wearable technology; community gardening projects; linkages with local food pantries; and virtual and in-person cooking demonstrations and grocery store tours.  Program referrals would come from the Health Department and Chesapeake Health Care.  The overall goals of the project are to promote healthier lifestyle choices (both exercise and nutrition) among the target population, weight loss, weight management, and improved food security.

Grant and Amount: 18-011 ($175,000)
Grant Project Description: This project would develop a new interdisciplinary chronic pain management center that would provide access to somatic health, behavioral health, and therapy services to help patients relieve chronic pain without the use of opioid medications. For those with Substance Use Disorders, the intervention offers a concentrated outpatient program using a multi-disciplinary approach to reduce or discontinue the use of opioids for pain management. This program is innovative and could be replicable in other areas of the state.

Grant and Amount: 18-019 ($175,000)
Grant Project Description: This project involves the enhancement of an existing Medication Assisted Therapy program through the addition of Naltrexone. The target population to be served includes: (1) Individuals released from inpatient addictions treatment programs; (2) inmates leaving Worcester County Detention Center with opioid addiction; and (3) individuals involved in Drug Court.

Grant Project Description: This two-year grant supports a program to increase access to behavioral health services in a rural and underserved area of the state by expanding the capacity of the organization to hire additional clinical staff. The program will address a current three-month waiting list for clients seeking services. Grant funds will be utilized to pay for the salary of a new clinical psychologist.

Grant Project Description: This three-year grant supported a program to improve access to somatic/primary care services for adults who have serious mental illness (SMI) and/or addictions illness. The new unit provides team-based care and access to publicly supported psychiatrists and therapists. Grant funds are being utilized to support staff salaries.

Grant and Amount: LHIC 13-003 ($250,000) 
Grant Project Description:  This two-year grant supported a program which targeted diabetes-related hospital ED visits and developed a comprehensive care coordination model to link frequent ED users with  access to primary care services in the community. The model leveraged community partnerships and addresses social determinants of health in the Lower Shore. Grant funds were utilized to develop and implement regional diabetes care management teams to assist diabetic patients in gaining access to a range of health care and social support resource

Grant and Amount: LHIC 12-002 ($25,000) 
Grant Project Description: This one year grant supported Worcester County Health Department’s efforts to  increase their capacity to address DHMH’s  State Health Improvement Process (SHIP) goals. SHIP focuses on improving population health outcomes and measures every jurisdiction on its performance on 39 population health metrics.

Grant and Amount: 08-021 ($355,000) 
Focus Area: Primary Care 
Grant Project Description: This two-year grant enabled Atlantic General Hospital to open a new behavioral health center to deliver services in an ambulatory care setting, targeting individuals using the hospital's emergency department for behavioral health issues. Grant funds were utilized to support personnel costs and to provide funds for capital improvements to open the new center. They also leveraged and additional $30,000 in private and federal funds to contribute to the sustainability of the program. Atlantic General reported a 30% decrease in emergency room use by patients utilizing the behavioral health care services at the Atlantic Health Center in the one-year after services were initialized.
