Description
A freestanding renal dialysis center is a facility that provides maintenance dialysis to chronic end stage renal disease patients on an outpatient basis. OHCQ is responsible for the licensure and regulation of freestanding renal dialysis centers, including the investigation of complaints.

Licensure
To apply for a license for a new dialysis center, complete the
Application for End State Dialysis Center License. In order to complete the application, please download a copy to your device. Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application. It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ. Submit the licensure application and all supporting documents to OHCQ through the
Submit a License Application link.
Certification
The Centers for Medicare & Medicaid Services (CMS) certifies dialysis centers. CMS refers complaints to OHCQ for investigation. To apply to be certified, visit CMS.gov and complete and submit the certification form.
