Public Health Testing Labs

Description

A Public Health Testing License is for laboratory testing that is affiliated with the Department for the purpose of offering or performing rapid HIV antibody testing or rapid hepatitis C virus testing approved by the Secretary.

Maryland Regulations

Licensure

To apply for a license for a Public Health Testing license, submit the ​Maryland State Public Health Testing Application​​​​​​​

There is no fee to apply for a Maryland State Public Health Testing license.  Please remember to include with the original signature mailed application, copies of the director's highest degree earned, medical license, and any board certifications.  After a completed application and all requested information is received by OHCQ, it takes 2 - 3 weeks to receive a license.
