Outpatient Physical Therapy Providers

​Description

An outpatient physical therapy provider (OPT) consists of a series of visits to a clinic in a hospital or to a stand-alone facility separate from a hospital.  Outpatient therapy is a course of treatment to support recovery and rehabilitation, delivered through a series of visits to a private therapy practice or a clinic.

OHCQ is responsible for the surveying OPTs for initial certification surveys, emergency preparedness review, recertification surveys, and complaint investigations. 

Regulations

Licensure

OHCQ does not license outpatient physical therapy providers. 

Certification

The Centers for Medicare & Medicaid Services (CMS) issues a certificate and refers complaints to OHCQ for investigation.  To apply to be certified, visit CMS.gov  and complete and submit the certification form.​
