Major Medical Equipment Providers

​Description

A major medical equipment provider is a facility using major medical equipment such as cardiac catheterization equipment necessary to perform heart catheterization; a computed tomography (CT) scanner; a lithotriptor; or radiation therapy equipment, including a linear accelerator; or a magnetic resonance imager (MRI).  It does not include any freestanding ambulatory care facilities or services owned or operated by a hospital and regulated under COMAR 10.07.01.

The Office of Health Care Quality (OHCQ) is responsible for licensure of major medical equipment providers.​

Regulations related to major medical equipment providers are found in COMAR 10.05.03. ​Licensure

To apply for an initial license or a change of ownership for a major medical equipment provider, complete the Licensure Application​.  In order to complete the application, please download a copy to your device.  Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application.  It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ.  Submit the Licensure Application and all supporting documents to OHCQ through the online system.  After a completed application and all requested information is received by OHCQ, it takes 3-6 months to issue a license.
