Limited Hospice Care Programs

Description

A limited hospice care program is a coordinated, interdisciplinary program of hospice care services for meeting the special physical, psychological, spiritual, and social needs of dying individuals and their families, by providing palliative and supportive nonskilled services through a home-based hospice care program during illness and bereavement.  Services are provided to individuals who have no reasonable prospect of cure as estimated by a physician and to the families of those individuals.

OHCQ oversees limited hospice care programs through licensure and complaint investigations.

Maryland Regulations

Licensure

To apply for a license for a new limited hospice care program, complete the Application for a Limited Hospice Care Program License.   In order to complete the application, please download a copy to your device.  Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application.  It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ.  Submit the licensure application and all supporting documents to OHCQ through the Submit a License Application​ link.

Licensed home health agencies report changes in their contact information to OHCQ via the Change Form​.
