Description
A general hospice is a coordinated, interdisciplinary program of of hospice care services designed to meet the special physical, psychological, spiritual, and social needs of dying individuals and their families by providing palliative and supportive medical, nursing, and other health-related services during illness and bereavement through home or inpatient care. OHCQ is responsible for the oversight of hospices, including initial licensure, recertification surveys, and complaint investigations.

Licensure
To apply for a license for a new hospice, complete the
Licensure Application. In order to complete the application, please download a copy to your device. Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application. It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ. Submit the licensure application and all supporting documents to OHCQ through the
online system.

Hospice Certification
To become eligible to open a licensed hospice in Maryland, you must first apply for a Certificate of Need (CON) from the Maryland Health Care Commission (MHCC). Learn more about the CON application process at the
