Description
A health care staff agency is any person, firm, corporation, partnership, or other business entity engaged in the business of referring health care practitioners as employees or independent contractors to render temporary health care services at a health care facility in the State.
OHCQ is responsible for the oversight of health care staff agencies, including licensure and the investigation of complaints.

Licensure
To apply for a license for a new health care staff agency, complete the
Licensure Application. In order to complete the application, please download a copy to your device. Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application. It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ. Submit the licensure application and all supporting documents to OHCQ through the
Submit a License Application system. After a completed application and all requested information is received by OHCQ, it generally takes 1 - 2 months to receive a license.
