Provider Adding New Sites

A licensed provider may add a new site by completing the DDA Application Process and submitting the required documentation.  After OHCQ receives all of the required and completed documents and DDA approves the new site, OHCQ will conduct a new site survey.  New site surveys are generally completed in 4 - 6 weeks.
