Description
A cosmetic surgery facility is a facility in which a cosmetic surgical procedure is performed. Cosmetic surgical procedure is the use of surgical services to reshape the structure of a human body to change the appearance of an individual. These facilities are designed exclusively for short-stay surgery. Cosmetic surgical procedure does not include a procedure done under local anesthesia or mild sedation, or liposuction that removes less than 1,000 cubic centimeters of aspirate. It does not include providers who are licensed as hospitals or ambulatory surgery centers. OHCQ is responsible for the licensure and oversight of cosmetic surgery facilities, including initial surveys, emergency preparedness review, and complaint investigations.

Licensure
To apply for a license for a new cosmetic surgery facility, complete the Licensure Application. In order to complete the application, please download a copy to your device. Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application. It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ. Submit the Licensure Application and all supporting documents to OHCQ through the online system.
