Comprehensive Outpatient Rehab Facilities

Description​

A comprehensive outpatient rehabilitation facility (CORF) is a medical facility that provides outpatient diagnostic, therapeutic, and restorative services for the rehabilitation of your injury, disability, or illness.  CORFs provide outpatient rehabilitation care, including medical care, therapy, and certain social or psychological services.  OHCQ is responsible for the licensure and oversight of CORFs.

Maryland Regulations  

Regulations related to comprehensive outpatient rehabilitation facilities are found in COMAR 10.07.18. Applying for a License

To apply for a license for a new comprehensive outpatient rehabilitation facility, complete the Licensure Application. In order to complete the application, please download a copy to your device.  Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application.  It is recommended that you save your progress throughout the process. Questions, comments or concerns regarding this application and its completion, can be referred to the associated program/unit within OHCQ. Submit the Licensure Application and all supporting documents to OHCQ through the online system​.  A CORF is also certified by the Centers for Medicare & Medicaid Services.
