​​Provider Information - Licensure

​The Office of Health Care Quality licenses Assisted Living Programs and oversees the quality of services provided to residents.  Prior to operating an Assisted Living Program in Maryland, the program must first obtain an Assisted Living Program license from OHCQ.  The first step in the licensure process is to complete the licensure application.  The application must be typed. In order to complete the application, please download a copy to your device.  Adobe Acrobat, Adobe Reader or another *.PDF application are required to complete the application.  It is recommended that you save your progress throughout the process.  Handwritten applications are not accepted and will be returned to the applicant.  Submit the completed application and all required attachments to OHCQ via the Submit a License Application link.

Report Changes Related to AL Staff

​​Licensed Assisted Living Providers use the forms below to report changes related to contact information for Assisted Living Managers (ALM), Alternate Assisted Living Managers (AALM), and Delegating Nurses:

​Report an Assisted Living Employee Who Becomes a Rep Payee

COMAR 10.07.14.35 requires that an Assisted Living Program must report to OHCQ each time an employee of the ALP becomes the representative payee (rep payee) of a resident.  Complete and submit this form to OHCQ every time an employee of your ALP becomes a representative payee for a resident. ​​

Report an Assisted Living Violation​​​

Per COMAR 10.07.14.08​, an Assisted Living Program must forward to the OHCQ a copy of any report or citation of a violation of any applicable building codes, sanitary codes, fire safety codes, or other regulations affecting the health, safety, or welfare of residents within 7 days of receipt of the report or citation.  Complete and submit the Report an Assisted Living Permit Violation form to OHCQ.
