HEART Payment

Beginning in January 2022, the MDPCP implemented the Health Equity Advancement Resource and Transformation (HEART) Payment. The HEART Payment provides additional support to MDPCP participants serving socioeconomically disadvantaged populations in order to promote the State’s and CMS’ goal to improve health equity.

Behavioral Health Resources

  • *NEW: Maryland 211 Behavioral Health Resource Database​

    • ​Maryland 211 and the MDH Behavioral Health Administration have launched this new Maryland 211 database that will help Marylanders find mental health and substance use disorder resources

Other Resources

  • Giant Nutrition - offers ​FREE online and phone consultations, online nutrition classes, workplace wellness webinars, community events and podcast to help Marylanders make the best decisions for their health and well-being. Visit Giant Nutrition or email nutrition@giantfood.com​ to learn more.

  • Giant Pharmacy - offers medication synchronization through the Time my Meds Medication Synchronization Program. After enrollment, patients can choose a fill date that all of their medications can be filled on. Patients can enroll by filling out this form​ and submitting it to the Giant Pharmacy where they get their prescriptions filled.

  • Staffing Resources - The following resources can be used by primary care practices to identify potential new staff to support their practices.
