This project will address disparities in heart disease and diabetes in the Capitol Heights, Bladensburg, Hyattsville, and Riverdale areas (zip codes 20710, 20737, 20743, and 20785). Key interventions include Community Health Worker (CHW)-driven outreach and care coordination, bi-directional e-referrals among health and social service providers, and technical assistance to improve providers' ability to bill for care coordination. The project will promote delivery of culturally and linguistically sensitive services, and utilize EMR, CRISP, and telehealth services including through telehealth hubs. Targeted outcomes include reductions in disparities related to heart disease and diabetes, improved diabetic control as measured by A1c or blood glucose levels, and increased access to primary care.

The program will address disparities in cardiovascular disease among low income and/or vulnerable Asian American residents in Baltimore County, Baltimore City, and Howard County. Asian American Health Center will provide one-stop comprehensive screenings (including EKG, A1c, cholesterol panel, lung disease screens) and a physician consultation to review test results, diagnoses, and medications, and offer referrals to primary/specialty care. In addition, care coordination services related to medication management and referrals for wraparound social/community services will be offered, as well as culturally and linguistically appropriate health education and health coaching.

Grant Project Description: This two-year grant enables Health Partners to begin billing for dental services and to allow the grantee to achieve long-term financial sustainability for its dental program. The grant builds on a past CHRC award to assist the clinic in transitioning from a grant-based revenue model to billing third-party payers for primary care services provided. Health Partners currently has two part-time dental hygienists and a volunteer dentist who is being credentialed as a Medicaid provider. The applicant proposes to transition their credentialed dentist (now volunteer) to a full-time dentist that can generate a fee-for-service revenue stream.
