Grant Project Description: The Cecil County Health Department will implement the Cecil County Diabetes Action Plan which aims to expand delivery of the evidence-based National Diabetes Plan to underserved, vulnerable low-income individuals whilst addressing common barriers to program recruitment and retention including lack of transportation and high medical expenses. Childcare vouchers will also be distributed to parents to encourage Diabetes Prevention Month attendance. The Cecil County Health Department plans to convene their Local Health Improvement Coalition (LHIC) as the coordinating body.

Grant Project Description: This project would provide services to low-income pretrial detainees involved with the Office of the Public Defender on misdemeanor or nonviolent felony charges who have a substance use disorder and lack access to appropriate treatment services in the community. The project would screen detainees and connect/serve them with treatment, support with a peer recovery specialist, and referral to other services as needed.

Grant Project Description: This project would provide integration of behavioral health and somatic care in Cecil County by placing psychotherapists in the offices of Union Primary Care, the largest primary care provider in Cecil County. The project implements the Screenings, Brief Interventions, and Referrals to Treatment (SBIRT Model), and the target population would be those who have substance use issues. This project would expand access to integrated behavioral health services in a rural and underserved area of the state.

Grant Project Description: This two-year grant is being used to implement an expanded dental program in Cecil County, a dentally underserved area of the state, through an innovative partnership involving the University of Maryland Dental School. Under a cooperative agreement, West Cecil has agreed to take over operations of the Dental School's clinic, which will maintain its status as a clinical teaching site with five pre-doctoral students and four hygiene students. Based on West Cecil’s status as a Federally Qualified Health Center, the program is designed to be sustainable after CHRC grant funds are expended. CHRC grant funds are being utilized to support staff salaries and supplies for the clinic.

Grant Project Description: This three-year grant supports the opening of a new Federally Qualified Health Center (FQHC) in Harford County. The new FQHC will offer primary care services in West Cecil in a Medically Underserved Area (MUA) between Aberdeen and Havre de Grace. Grant funds are being utilized to support the start-up operational costs of the center. West Cecil Community Center leveraged this grant funding to receive an additional $250,000 from the Weinberg Foundation.

Grant Project Description: This one year grant supported the Cecil County Community Health Advisory Committee program aimed at the reduction of behavioral health-related visits ED visits. This assessment will analyze multiple factors involving prescription abuse and will provide a blueprint for suggested potential effective interventions for the continuum of services and agencies involved that will guide Cecil County’s work in this area for several years.

Grant Project Description: This three-year grant was awarded to integrate behavioral health care services at its primary care facility. The behavioral health care team consults with WCHC’s primary care providers to refer patients to services and work collaboratively to ensure comprehensive, integrated care to patients who need primary and behavioral health care services. Grant funds were utilized to support the salaries of behavioral health practitioners. The grantee reported serving 4645 patients through 11,515 visits over the duration of the grant.
