Grant Project Description: Chesapeake Cares Food Pantry, the largest food pantry in Southern Maryland, will implement the Eat Smart, Move More Calvert! pilot program which seeks to serve low-income food pantry clients with diabetes and/or pre-diabetes. CHRC grant funding will support the hiring of a Food Ambassador to develop a team of volunteer health coaches and provide for cooking classes and food distribution costs. This program will also leverage multiple health resources and community supports for participants. Multiple services pertinent to the client for achieving health and addressing other social determinants of health are provided at no cost.

Grant Project Description: This two-year grant supports an innovative re-entry program led by the Calvert County Health Department, the sole safety net provider of behavioral health services in Calvert County. The project addresses the social determinants of health impacting formerly incarcerated individuals and looks to develop concrete measurable outcomes to track/demonstrate the performance of re-entry programs at the local level. The program has the potential to serve as a model for local re-entry efforts in other areas of the state. Grant funds will be utilized for staff salaries and the provision of social service supports for program participants.

Grant Project Description: This three-year grant supports a program to reduce infant mortality rates by creating a new, “one-stop shop” of integrated behavioral health and social services for substance abusing women and expectant mothers. Grant funds are being utilized to develop and implement the multi-disciplinary program which includes intensive case management and linkage to local obstetric providers, family planning, folic acid supplements, behavioral health services, WIC, social services, dental care, health insurance enrollment, and community resources such as education and job training opportunities.

Grant Project Description: The Calvert County Health Improvement Roundtable utilized grant funds to form a collaborative, comprehensive community care coordination team with faith and community-based organizations and local health system providers to target a reduction in diabetes emergency room visits by the African American population. CHRC grant funding supports development of a “Diabetes Boot Camp” designed to educate patients about monitoring blood sugar levels and maintaining normal blood sugar levels.

Grant Project Description: This three-year grant expanded the grantee’s capacity to provide primary health care services and linkage to other service supports in Calvert County. The grant supported an increase in service hours for primary care and mental health services, the creation of a formal referral consortium with community agencies, and an increase in access to prescription assistance programs. Grant funds were utilized to support salaries for expanding the program. The grantee reported serving 493 patients through 3,311 visits over the duration of the grant.

Grant Project Description: This three-year grant supported improving access to health care services for low-income and uninsured residents of Calvert County by increasing the capacity of the Twin Beaches Community Health Center, increasing access to the case management and providing supplemental payments to specialists and an area pharmacy to cover the gap between patients sliding fee scale payments and actual costs. This program provided services to 898 patients with 6,500 visits over the duration of the grant.
