    Transformation Plan

On July 8, 2015, the Deputy Secretary issued the Developmental Disabilities Transformation and Communication letter to stakeholders to transform policies and funding processes to create a flexible, person-centered, family-oriented system of supports so people can have full lives.

We are committed to doing business efficiently, effectively, and professionally by strengthening the community and enhancing our service delivery across the state. The DDA’s community-based service delivery systems includes various home and community-based services and supports provided through either a Medicaid State Plan or Waiver program or via DDA State funded services.

Our transformation will take some time as reflected in the projected timeline. With assistance and support of individuals, family members, and other stakeholders, the current system will transform into one that supports people with developmental disabilities to have full lives in the communities of their choice where they are included, participate, and are active citizens.

The DDA has created this webpage to inform you of our transformation and to keep you informed of our progress and new developments so that participants, families, providers and our community can have the most current information about our transformation and our commitment to transparency.
