Keeping Mental Health and Substance Use Programs Safe and Strong


One of the Behavioral Health Administration responsibilities is to identify and prevent fraud, waste and abuse. These efforts help ensure your loved ones have access to quality care, support legitimate healthcare providers, and protect Maryland taxpayer dollars so they can be used to improve the health and well-being of Marylanders.

Need to file a complaint? 
If you have concerns about a behavioral health care provider or treatment (mental health or substance use disorder) that you or a family member received or did not receive, you can complete this form

  • Individual behavior health counselor: To file a complaint about care provided by an individual behavior health counselor or physician, please contact the individual's licensing board.  
  • Hospital, federally qualified health center, or residential treatment facility: To file a complaint about care provided at a hospital, federally qualified health center, or residential treatment facility, please contact the Office of Health Care Quality.   
  • Medicaid Fraud: To report Medicaid fraud, waste or abuse call the Office of Inspector General (OIG) Hotline: 866-770-7175 or submit the online form: Report Fraud.
The Maryland Department of Health (MDH) Office of the Inspector General ​handles all reports sent to the hotline or online form.
