Need to file a complaint? Complete this form if you have concerns about a behavioral health care provider or treatment (mental health or substance use disorder) that you or a family member received or did not receive.

To file a complaint about care provided by an individual behavior health counselor or physician, please contact the individual's licensing board.
To file a complaint about care provided at a hospital, federally qualified health center, or residential treatment facility, please contact the Office of Health Care Quality.
