Birth Certificates can be obtained for individuals born in Maryland for a fee of $25 at the Health Department, Monday through Friday, 8:30 a.m. to 4:00 p.m., except State Holidays.
Birth certificates may be requested in person or online. You will be contacted to schedule a pickup time for your certificate, or it will be mailed to you.
We accept cash ($20 bills or smaller), checks, Visa, MasterCard, or Discover.
How to apply:
You will need a current, government-issued photo ID that includes an issue date and expiration date.
If you do not have a current government-issued photo ID, the certificate will be mailed to the address listed on two of the following documents that you present as identification. The address on both documents must be the same.
- Pay stub
- W2
- Rental/Lease agreement
- Bank statement
- Utility bill
- Car registration
In-person instructions:
If possible, download and complete the Birth Certificate Application and bring the completed form with you.
Present a valid goverment-issued photo ID or verification documents to the clerk.
A copy of a birth record may only be issued to:
- the person named on the Certificate;
- a parent or court-appointed guardian;
- a representative with a notarized letter signed by the person named on the Certificate or a parent or guardian granting permission to obtain a Certificate;
- a surviving spouse, an individual with a court order directing that the Certificate be issued;
- or an individual permitted to obtain a certificate under Md. Code Ann., Family Law Title 5, Subtitles 3A or 4B relating to adoptions.
Online instructions:
Complete the online application and pay by credit card.
Once the application is received, someone from our office will contact you about picking up the completed document.
Note: please be sure to complete the application accurately and upload a picture of your government-issued photo ID *or* two of the above listed documents. Failure to do so will delay processing of your application.
