Notice: On December 4, 2021 a security incident occurred involving the Maryland Department of Health (MDH) information technology systems. Out of an abundance of caution, MDH servers are currently offline. As a result, we are not able to enter new applications or retrieve license/permit renewal information at this time. This has delayed the processing of all new applications for licenses, plan reviews, and license/permit renewals. If your license needs to be renewed, and you have not received your renewal letter, please submit last year’s application, mark in the top right corner it is for a 2022 renewal, and re-sign and re-date the application. Applications along with payment should be mailed to the Office of Food Protection, Permits and Licensing, 6 St. Paul St. Suite 1301, Baltimore, MD 21202. Notice: The Governor’s executive order which extended the expiration date of all permits, licenses, and registrations during the COVID-19 public health emergency is no longer in effect. All businesses are required to have a current license to be approved to operate. Licensing and plan review fees have been reinstated as of July 1, 2021.
