Community Options Waiver

The ​​Community Options Waiver, also as known as the Home and Community-Based Options Waiver (HCBOW), is a home and community-based services waiver. It helps you get services and support in your home and community instead of in an institution.

The Maryland Department of Health Medicaid Administration runs this waiver program. This is known as the Operating State Agency (OSA). The OSA focuses on serving the people in the waiver and manages specific waiver services.

Am I eligible for the Community Options Waiver?

You must meet all the medical, technical and financial rules to be eligible for this waiver.

  • Medical Eligibility Rules:
    • Need nursing facility level of care.
  • TechnicalEligibility Rules:
    • Be age 18 and older.
    • Live at home or in the community.
    • Not be in another waiver program or the Programs of All-Inclusive Care for the Elderly (PACE) at the same time.
    • The cost of community-based services must not be more than institutional care.
    • Be able to live safely in your home and community with these waiver services.
  • Financial Eligibility Rules:
    • Based on income and assets of the household.
    • Monthly income may not exceed 300% of SSI benefits.
    • Countable assets may not be more than $2,000 or $2,500, depending on eligibility category.

If Medicaid denies your application because your income is too high, you may be eligible for the Increased Community Services Program​.

What services does the Community Options Waiver provide?

You get all Medicaid covered services and benefits including the Community First Choice Program​. You also may get waiver services that include:

  • Assisted living services
  • Behavior consultation 
  • Case management
  • Dietitian and nutritionist 
  • Family training
  • Medical day care
  •  Senior Center Plus

 How to Apply

  • There is a waitlist for this waiver. Call Maryland Access Point​ at 844-627-5465 to get your name on the Community Options Waiver Registry. 
  • You will receive an invitation to apply when it is your turn.
If you are in a nursing facility and Medicaid has paid for services for at least 30 days, you may apply with no wait. Talk to your nursing facility’s social worker to learn more. You may ask for a referral to an options counselor for help with the application. See contact information below.
