REM Program

​The Rare and Expensive Case Management (REM) program provides medical case management and other services for eligible individuals with specific medical conditions. This is a voluntary program.

You will have an REM case manager who will help you make a plan of care and to find the services you need.

Your services must be medically necessary, ordered by your health care provider, and may require preauthorization. Your REM case manager can help you with questions. 

Eligibility

 To be eligible for the REM Program, you must:

REM covers:
  • Case management assessment and services
  • Certified Nursing Assistant (CNA) and CNA Certified Medication Technician (CMT) services
  • Home Health Aide (HHA) and HHA Certified Medication Technician (CMT) services
Some REM services are only for those 21+:
  • Chiropractic services
  • Nutrition counseling and supplements
  • Occupational therapy
  • Speech-language pathology
  • Private duty nursing, shift nursing services

Apply

Anyone can refer you to the program. A licensed health care provider must fill out part of the Intake form, sign, and date it. See the REM Intake and Referral Form instructions for more information.
