Rare and Expensive Case Management (REM) Program

​The Rare and Expensive Case Management (REM) program provides medical case management and other services for eligible individuals with specific medical conditions. This is a voluntary program.

Your services must be medically necessary, ordered by your health care provider, and may require preauthorization. You will have an REM case manager who will help you make a plan of care and to find the services you need. Your REM case manager can help you with questions.

You get all Medicaid covered services and benefits. You also may get REM services that include:

Case Management Services

  • Conduct a needs assessment.
  • Develop a care plan with you and those involved with your care.
  • Help you apply for services.
  • Keep an eye on your services and make changes to your care plan as needed.
  • Provide education and information.
Other Services

  • Certified Nursing Assistant (CNA) and CNA Certified Medication Technician (CMT) services
  • Chiropractic services
  • Home Health Aide (HHA) and HHA Certified Medication Technician (CMT) services
  • Nutrition counseling and supplements 
  • Occupational therapy
  • Speech-language pathology 
  • Private duty nursing, shift nursing services benefit.

Apply

Anyone can refer you to the program. A licensed health care provider must fill out part of the Intake form, sign, and date it. See the REM Intake and Referral Form instructions for more information.
