Effective October 1, 2023, the Collaborative Care Model is now available to all Maryland Medicaid enrollees.
The Collaborative Care pilot program officially sunset on September 30, 2023 (see below).
Provider Information
The Collaborative Care Model is a patient-centered, evidence-based approach to integrate physical and behavioral health services in primary care settings. Primary care provider (PCP)-led teams of qualified professionals are eligible to receive reimbursement for CoCM services. These teams must include a PCP, a behavioral health care manager, and a psychiatric consultant. CoCM includes:
- Care coordination and management between your Primary Care Provider (PCP) and your behavioral health provider;
- Regular, systematic monitoring and treatment using a validated clinical rating scale; and
- Regular, systematic psychiatric caseload reviews and consultation for patients who do not show clinical improvement
Participating Providers
Reimbursement for CoCM services will be billed under the lead provider’s name and provider number.
