​​Social Determinants of Health (SDOH)​

​​​​The Social Determinants of Health program is our newest barrier removal program. This program empowers the community to self-identify the obstacles caused by adverse determinants that widen the gap in health disparities and are the primary factors in the poor health outcomes of minority communities in the state of Maryland. ​

Grantees for fiscal year 2023 (July 2022 - June 2023) will concentrate on the following focal areas and provide services within the specified counties.  A complete list of the organizations can be found by clicking here. ​​
