Community Pathways Federally Approved Application/Amendments
All Medicaid Waiver programs must be approved by the federal Centers for Medicare and Medicaid Services (CMS). Once approved they are effective for a five-year period unless amended or discontinued. Requests to make changes to a Waiver program are made by submitting a Waiver Amendment to CMS.
