It is almost time to renew your Crematory Operator Registration, Crematory Permit, and/or Mortuary Transport Permit.
NOTE: The Board is now accepting online renewal applications and payments. Electronic payments are a convenience for Licensees and improves the efficiency of the services provided by the Board. Board staff is excited about the progress made in this area of the operation and is appreciative of your FULL cooperation.
The online licensing renewal application is available for reviews and approvals starting August 1, 2024.
A late fee will be assessed if the Board does not receive your renewal application on or before the expiration date indicated on your renewal application.
All of the listed permits and registration above expire September 30th every two years.

If your name or address has changed, note the new address and/or enclose proof of name change, such as a court document or marriage certificate.
Please pay the requested Renewal Fee using the RENEWAL PAYMENT LINK located on the application. The Board has the authority to charge a late fee; if your license renewal application is not received on or before your designated expiration date. In addition to the regular renewal payment; please use the LATE FEE PAYMENT LINK.

***An individual who engages in the operation of a business or acts as a licensee without renewing the required permit or registration is subject to disciplinary action by the Board.

non-renewed Mortician Licenses for the renewal cycle ending April 30, 2024. If your name is on this list contact the Board's Licensing Coordinator to access the online renewal system and renew your expired Mortician License ASAP at 410-764-3453 or 443-934-3539. There will be a late fee of $400 applied. **You are considered non-renewed and you are NOT permitted to practice mortuary science until your application and fees are received and approved.**
