This list relefts all licensees non- renewed as of October 15, 2024. In addition to the renewal fee you must pay the late fee of either $200 or $400. *** (If applicable) DO NOT forget to schedule your Mortuary Transport Vehicle Renewal Inspection at the following link https://app.smartsheet.com/b/form/26e8424a5c3248b0aa897c9c5bd6ee21 Once your renewal application has been approved the Investigator/Inspector will contact you to schedule an inspection date and time. **You are considered non-renewed and you are NOT permitted to practice mortuary science until your application and fees are received and approved.**

It is time to renew your Funeral Establishment and/or Corporate License.
The online licensing renewal application is available for reviews and approvals starting November 1, 2024. A late fee will be assessed if the Board does not receive your renewal application on or before December 15th, 2024.
All Funeral Establishment and Corporate Licenses expire November 30th every two years.
Please renew your license online at the following link https://app.smartsheet.com/b/form/850661645a764878990f9ed366a8c8b7 listed on our website under ONLINE SERVICES.
If your name or address has changed, note the new address and/or enclose proof of name change, such as a court document or marriage certificate.
Please pay the requested Renewal Fee using the RENEWAL PAYMENT LINK located on the application. The Board has the authority to charge a late fee; if your license renewal application is not received on or before December 15, 2024. In addition to the regular renewal payment; please use the LATE FEE PAYMENT LINK if you submit your application after December 15, 2024.
***An individual who engages in the operation of a business or acts as a licensee without renewing the required license is subject to disciplinary action by the Board.

If your name is on this list contact the Board's Licensing Coordinator to access the online renewal system and renew your expired Mortician License ASAP at 410-764-3453 or 443-934-3539. There will be a late fee of $400 applied. **You are considered non-renewed and you are NOT permitted to practice mortuary science until your application and fees are received and approved.**
