The Children’s Medical Services Program, also called the CMS Program, is a program of the Maryland Department of Health. CMS can help low-income, uninsured or underinsured Marylanders, ages 0-21 years, get specialty care for a chronic illness or disability by paying for some needed services. ​ ​CMS can pay for many kinds of services, such as visits to a specialist, hospitalizations, surgeries, medications, therapies, lab work, x-rays, and medical equipment/supplies. CMS does not cover routine check-ups or immunizations, sick visits, emergency services, routine dental care, mental health services, transportation, and other services. 

In order to be covered by the CMS Program, all services must be pre-authorized. CMS requires a minimum of 10 business days to process the pre-authorization, before the service is rendered. 

Electronic Forms / Emails
CMS is now incorporating Cognito Forms for electronic submission of
Applications, Pre-Authorization requests and Claim Submissions.

 CMS Affidavits

Families can now complete one of the below corresponding Affidavits, instead of notarized letters. The family will need to provide a notarized letter, only if requested by CMS staff.
