Under the rule's Payment Adequacy provision, states must track the percentage of Medicaid payments that go directly to direct care workers. Maryland Medicaid will use this data to set fair, data-driven reimbursement rates and ensure long-term program sustainability.

Note: Maryland Medicaid will suspend payments to providers that do not submit the survey by Dec. 31, 2026. Payments will resume once Medicaid receives the completed survey.
Who Needs to Participate?
All Medicaid-enrolled HCBS providers that delivered non-Developmental Disability Administration (DDA) services during state fiscal year 2026 must complete the survey. This requirement applies regardless of whether CMS specifically mandates reporting for your service category.
Who is a Direct Care Worker?
A HCBS direct care worker is any person you employ or contract to provide hands-on services to Medicaid members. This includes workers who provide personal care, home health, or habilitation services. Roles include:
- Direct Care Staff: Personal care aides, nursing assistants, home health aides, direct support professionals, and home care aides.
- Clinical Staff: Registered nurses, licensed practical nurses, nurse practitioners, and clinical nurse specialists who provide direct services.
- Supervisors: Staff who provide clinical supervision to the direct care workers.
