Maryland Medicaid offers home and community-based services for:
- Older adults;
- Persons with disabilities; and
- Children with chronic illnesses.
Maryland Medicaid’s home and community-based services are offered through the regular Medicaid program and special Medicaid programs called “waivers.” Each waiver has different eligibility criteria and target a different population, such as older adults or people with a certain disability.

Community Pathways Waiver, Community Supports Waiver, and Family Supports Waiver
The Maryland Department of Health is amending the 1915(c) waiver application for the Medicaid waiver operated by the Developmental Disabilities Administration (DDA)—the Community Pathways Waiver (CPW). This amendment aims to consolidate the three existing Medicaid waivers operated by the DDA—the Community Supports Waiver and the Family Supports Waiver—into a single, comprehensive Community Pathways Waiver.
The Medicaid waiver program provides various home and community-based services to support participants as they work toward individually defined life outcomes, self-determination, community living, socialization, and economic self-sufficiency.

The Notice of Request for Public Comment has also been posted at the local DDA Regional Offices. Hard copies of the proposed updates are available for public review and comment via request submitted to wfb.dda@maryland.gov.

*Due to technical difficulties with uploads to the DDA website, the proposed amendments for Respite Services and Supported Living Services were not posted on June 9, 2025, as originally scheduled.
To ensure all stakeholders have adequate time to review and provide feedback, the public comment period for Respite Services and Supported Living Services services will be extended until July 13, 2025.
Waiver for Adults with Brain Injury Waiver — AMENDMENT APPLICATION: Request for Public Comment
The Maryland Department of Health (MDH) will submit a 1915(c) Home and Community-Based Services Waiver amendment application for the Waiver for Adults with Brain Injury (Brain Injury Waiver) to the Centers for Medicare and Medicaid Services (CMS) to amend Brain Injury Waiver effective September 1, 2025. The Brain Injury Waiver provides various home and community-based services to support program participants and enable them to remain in their homes and communities. All services must be provided in settings that meet federal home and community-based settings requirements.
