Money Follows the Person

​Money Follows the Person (MFP) is a Medicaid program that lets you choose where to live and to get long term care services if you are eligible. The program may help you move into the community. Your Medicaid long-term care can “follow the person”, you, to​ a home, an apartment or a group home with four or less people. 

A team, with a peer mentor, can help you find the housing, car​e and servi​​ces you need to move and to live on your own.

Eligibility

To be eligible for the Money Follows the Person Program, you must:​

  • ​​​Live 60 days in one of the following:
    • A nursing home, or
    • An intermediate care facility for individuals with intellectual and developmental disabilities. 
    • Have one day of Medicaid eligibility durning your stay.
  • ​Sign an MFP Consent Form​.
