Our Provider Finder makes it easy to find the care you need. You can search for a doctor, specialist, hospital, or other facility near you. All providers and facilities in the Provider Finder accept Medicaid. Not all accept every
HealthChoice plan.
How to Use the Provider Finder
- Select a HealthChoice plan if you only want to find a provider or facility that takes that plan. Otherwise leave this blank.
- Enter a location for your search.
- Choose a distance if you want to limit how far away you will go.
- Enter a provider or facility name or pick a provider type or specialty from the drop down to search by the type of care.
- Click on the search button.
- See your results by provider and facility. Click on the number next to each of these words to see only those results.
Other Provider Finder Features
- Use the filters to narrow your search based on your needs.
- Click on show more details to see more information and a map of the location.
- Click on the map to get directions.
- Click on the print results button to print the information from your search.
Are you a HealthChoice member or looking for a HealthChoice provider? You can use the Provider Finder tool for a quick search. This tool shows only:
- The provider’s name.
- The provider’s primary office location.
- The HealthChoice plans they accept.
To find all office locations, see if a provider is accepting new patients, or check their specialty, visit the HealthChoice plan's directory.
